George Michalopoulos, D.C.Was born in Toronto, Ontario, Canada.He did his undergraduate studies in Queen’s University in Kingston, Ontario.Dr. Michalopoulos completed his undergraduate studies at the National College of Chiropractic in Lombard, Illinois receiving his bachelor’s degree in Human Biology. He completed his Doctorate of Chiropractic in December 1989.

 Dr. Michalopoulos served his internship at the Brookfield Clinic in Brookfield, Illinois.He is active in many sports, including softball, baseball and hockey.Dr. Michalopoulos is married to his wife Kathy of 17 years and has three beautiful daughters Christina, Eleni and Alexandra.His family resides in Itasca, Illinois.

 Dr. Michalopoulos has been in practice for 17 years and believes in restoring patient’s optimal health in the shortest period of time as possible, through multidisciplinarycare including,  rehabilitation, exercise, diet, and education of the patient’s existing problems. He is currently working on his electro diagnois and subsequent neurology diplomat.

 He has dedicated much of his time and effort to educate industry and community in the care and prevention of low back, neck, and work and sports injuries.  He has treated many professional hockey players from the National Hockey League and is active in the charitable organization of the Chicago Blackhawks hockey club.  Professional memberships and charity organizations include, Illinois Chiropractic Society, Society for Pain and Practice Management, Multi-disciplinary Academy of Affiliated Medical Arts (M.A.A.M.A.), Chicago Blackhawks Hockey Club Alumni Association, Addison Chamber of Commerce, Wellness Council Of America, and has a certificate of completion in Successful Health Management Systems from Wellsource.

Christine Karchmar, D.C. attended East Tennessee State University where she received her Bachelor of Arts degree in Chemistry and French. She received her Doctorate of Chiropractic degree and Bachelor of Science degree in Human Biology from the National College of Chiropractic in Lombard, Illinois in December 1999.

 Dr. Karchmar and husband reside locally in the western suburbs.

 Dr. Karchmar has practiced as an associate for the past 8 years.She strongly believes that Chiropractic care and nutritional support go hand in hand. Patient education is one of the most important aspects in helping patients live a happier healthier life. Dr. Karchmar is licensed in Acupuncture as well.

 Professional memberships include, American Chiropractic Association, Illinois Chiropractic Society, AmericanAcademy of Spine Physicians, and the American Back Society.

Martin G. Fortier, M.D.was born in Chicago and raised in Roselle, Illinois. He completed undergraduate studies at the University of Colorado. Upon completing medical school at Rush Medical College in Chicago, Illinois, Dr. Fortier received residency training at the University of Chicago, Christ Medical Center, and the Cook County Hospitals and Clinics. Dr. Fortier is board certified by the AmericanCollege of Emergency Physicians. In addition to the practice of Emergency Medicine, Dr. Fortier has extensive experience in the practice of Occupational and Sports Medicine.

 Dr. Fortier has practiced Emergency and Occupational Medicine in the Chicagoland area for almost 20 years.His goal as a member of the Addison Physical Medicine team is to assist in the recovery and rehabilitation of injuries whether they are trauma, job, or sports related.Additional professional memberships include the American Medical Association, Illinois College of Emergency Physicians, and the Chicago Medical Society.

Pretzel Amadora Ceniza, L.P.T. graduated from Velez College with a Bachelors of Science degree in Physical Therapy in 2001.

 Pretzel served her internship in different settings; in-patient and out-patient hospital settings, out-patient rehab centers, community based rehab, nursing homes, and home health rehab.

 Pretzel is married to her husband Romeo, who is also a licensed physical therapist and reside in Elgin, Illinois.

 Pretzel is an innovative physical therapist specializing in work-related injuries, sports injuries, and musculoskeletal disorders.Pretzel firmly believes that quality of patient care is the key towards over-all improvement.

Gina Reveliotis, Office Manager was born in Toronto, Ontario Canada and is a Graduate of University of Ottawa with a Bachelor of Arts degree in social Sciences.

Gina is the office manager at Addison Physical Medicine.  In addition Gina is the billing & finance manager. Her many duties include interacting with numerous medical facilities and insurance companies each day, verifying insurance benefits, and assisting patients in making sure they receive the proper care necessary. She trains and supervises the staff members so that they can provide each patient with the best quality health care. Gina prides herself on her vast knowledge, courteous service, and her excellent multi-task skills. Gina is also involved in the coordination of Addison Physical Medicine's worksite wellness programs.  She works along side the corporate wellness director in providing companies a solution to rising insurance premiums.

Gina is married to her husband George and resides in the Northwest suburbs of Illinois.
